Please complete the New Patient Registration forms listed below after we have booked an appointment for you. This will save time during your first visit and ensure that all information is accurate.

The Good Faith Estimate:

As of January 1, 2022, all healthcare providers are required to provide estimates for the costs of you care if you request it. The Good Faith Estimate shows the cost of items and services that are reasonably expected for your healthcare needs and treatment. Our physical therapy practice has transparent pricing with one fee per visit that is paid at the time of treatment, and no other costs. However, you can now receive an additional document per the new law, termed the Good Faith Estimate, upon scheduling and/or when requested that indicates this.
